For your child’s/children’s safety when attending one of our workshops, we ask that parents provide the following information. Rest assured that all materials used at ArtShops are non-toxic, and your children will be supervised at all times. We will make every effort to contact you or your alternative contact in the event of an emergency. Please bring with you to the next event. We can keep it on file for future workshops. Also, feel free to have the form notarized (banks often provide this service for free). Thanks for cooperating with us to provide a safe place for your kids to learn!
